About Our Practice

Our dedicated team of primary care physicians and nurse practitioners, behavioral health experts, and care coordinators—works together so you have the care you need. From sick visits to preventive care and specialty services for chronic conditions, we coordinate care to meet every patient’s unique needs. Our goal is for you to be as healthy as possible. 

Our patients have easy access to Southern New Hampshire Health’s network of distinguished specialty care providers and services, as well as our state-of-the-art medical center. We’re also located right next door to Immediate Care, with 7-day access to care for urgent needs, as well as lab and X-ray.

Our practice offers the best of both worlds: convenient care in your community, backed by the world-class resources of Southern New Hampshire Health and our clinical affiliate Massachusetts General Hospital.
