How to Apply for Financial Assistance
Call our financial counselors at (603) 281-6630. Our financial counselors can assist you with finding health care programs you may be eligible for. They can help you apply for financial assistance, Federal and State based programs and other healthcare funding sources.
Financial Assistance Applications
Paper applications and a hard copy of our financial assistance policy are available at the cashiers office located near the main lobby of Southern New Hampshire Medical Center, 8 Prospect Street, Nashua. The cashier office is open Monday through Friday, 8:00 am to 4:00 pm or you can call us at (603) 281-6630. Send a request to this address or call this number if you would like an application and/or a copy of our policy mailed to you.
Charitable Care Policy
Southern New Hampshire Health is committed to providing charitable care to persons who have health care coverage needs. This includes persons that are uninsured, underinsured, are ineligible for governmental programs and are otherwise unable to pay for medically necessary care based on their individual financial situations. Get Help Paying for Prescriptions
If you’re uninsured, under-insured or elderly, we may be able to help you get free or reduced-cost prescriptions. We’ll work with you to apply for programs provided by pharmaceutical companies for discounted prescriptions. The Granite Advantage Healthcare Program (GAHP)
The Granite Advantage Healthcare Program is available to help low income individuals in New Hampshire who do not have medical insurance coverage. If your income is less than 138% of the Federal Poverty Level (see chart below) and you are between the ages of 19 and 65, you may be eligible for this new benefit. Once you are approved for the GAHP, you will have access to primary care, preventative health services, specialist services and prescription coverage!
There are four ways to apply:
1. Online: visit nheasy.nh.gov
Hours: 6:00AM and 12:00AM (midnight) 7 days a week
2. By phone: Call the Medicaid Service Center at 1-888-901-4999 where you will receive
personalized assistance and can get your questions answered
Hours: 8:00AM to 4:00 PM Monday - Friday
3. Submit a paper application: Visit a DHHS District Office or download an
application here on our website http://www.dhhs.nh.gov/dfa/apply.htm
4. The ACA Marketplace: Go online and sign up at www.healthcare.gov
