We are here to support you, offering expert symptom management, emotional support, and guidance in making informed healthcare decisions.

We offer both inpatient and outpatient care:

• Outpatient care is offered at Foundation Palliative & Supportive Care, which includes evaluations, follow-up visits and other care as needed provided through the palliative care team.
• Inpatient care is offered for patients who are admitted at Southern New Hampshire Medical Center.
