We’re accepting new patients. To make an appointment:
  • Call (603) 888-5573 during business hours (Monday to Friday, 8am to 5pm)
  • To schedule an appointment online, please log into your MyChart account.
  • Are you an established patient at Southern NH Health or The Elliot and not currently enrolled? If so, click here to sign-up and start scheduling online appointments today.
To get a same-day appointment, call us as early in the day as possible. Should you require immediate, non-life-threatening medical attention, please call us. In those instances when we are not able to see you, we may refer you to Immediate Care.
 
Please arrive 10 to 15 minutes before your appointment to fill out paperwork. To transfer your medical records, print these forms and bring them to your appointment:

After-Hours Care

When our office is closed, you can reach an on-call provider by calling our office at (603) 888-5573.
 
For life-threatening emergencies, like chest pain or shortness of breath, call 911 or go to the nearest Emergency Department. We also offer walk-in 7-days a week at Immediate Care.
 

Prescription Refills

You can usually refill your prescription at your visit, or you can use the patient portal,or call our office at (603) 578-7415
 
Please allow 24 to 48 hours for your refill. Some medicines may take longer because we need to get a prior authorization from your insurance company.
 

Referrals

Many health plans require a referral from your doctor for other services like specialty care. To get a referral, call our office at (603) 888-5573. Please allow 5 to 7 business days for your referral. Unless you are having a life-threatening emergency, please call our office first before getting medical care elsewhere.
 

Health Care Forms

If your physical exams and vaccines are current, you can get this information through your patient portal for school, camp or other needs. If you have trouble accessing this information, please contact us, and we will send your forms within 3 to 5 business days.
