To make an appointment:
  • Call (603) 577-3230 during business hours (Monday to Friday, 8:00am to 5pm)
  • To schedule an appointment online, please log into your MyChart account.
  • Are you an established patient at Southern NH Health or The Elliot and not currently enrolled? If so, click here to sign-up and start scheduling online appointments today.
Please arrive 10 to 15 minutes before your appointment. If you are a new patient, please plan to arrive 30 minutes prior to your appointment. If your insurance company requires a co-pay, we will ask you to pay it at this time.
 

After-Hours Care

When our office is closed, a designated provider is on-call to answer your questions and guide you to appropriate care. If your problem is not life threatening, always call our office at (603) 577-3230.

For life-threatening emergencies, like chest pain or shortness of breath, call 911 or go to the nearest Emergency Department. We also offer walk-in non-emergency care 7-days a week at Immediate Care.

Prescription Refills

You can usually refill your prescription at your visit. Or you can use the patient portal or call our office at (603) 577-3230.      
 
Please allow 24 to 48 hours for your refill. Some medicines may take longer because we need to get a prior authorization from your insurance company.
 

Referrals

Most insurance companies require you to obtain a referral from your primary care doctor before being seen by a specialist. Contact your doctor to request a referral prior to your visit with us.
