To make an appointment:
  • Call (603) 886-7900 during business hours (Monday to Friday, 8:00am to 5pm)
  • If you are an existing patient and feel ill, we will determine if you need to see a specialist in our practice or your primary care doctor.
  • If you are a new patient and not part a patient of a Foundation Medical partners practice,  to transfer your medical records, print these forms and bring them to your appointment: Release of Records to Foundation Hematology/Oncology

After-Hours Care

When our office is closed, you can reach an on-call provider by calling our office at (603) 886-7900.

For life-threatening emergencies, like chest pain or shortness of breath, call 911 or go to the nearest Emergency Department. We also offer walk-in non-emergency care for 7-days a week at Immediate Care.

Prescription Refills

You can usually refill your prescription at your visit. Or you can use the patient portal or call our office at 603) 886-7900.      
 
Please allow 24 to 48 hours for your refill. Some medicines may take longer because we need to get a prior authorization from your insurance company.
 

Referrals

Many health plans require a referral from your doctor for other services like specialty care. Please contact your doctor to request a referral prior to your visit with us.
