To make an appointment:
  • Call (603) 577-3322 during regular business hours.
  • To schedule an appointment online, please log into your MyChart account.
  • Are you an established patient at Southern NH Health or The Elliot and not currently enrolled? If so, click here to sign-up and start scheduling online appointments today.
  • To get a same-day appointment, call us as early in the day as possible. 
If you are a new patient, plan to arrive at least 10 to 15 minutes before your appointment.

After-Hours Care

When our office is closed, you can reach an on-call provider by calling our office at (603) 577-3322.

For life-threatening emergencies, like chest pain or shortness of breath, call 911 or go to the nearest Emergency Department. We also offer walk-in care 7-days a week at Immediate Care.
 

Prescription Refills

You can usually refill your prescription at your visit. Or you can use the patient portal or call our office at (603) 577-3322.       

Please allow 24 to 48 hours for your refill. Some medicines may take longer because we need to get a prior authorization from your insurance company.
 

Referrals

Many health plans require you to obtain a referral from your primary care provider before seeing a specialist. Please contact your provider’s office to request a referral prior to your visit with us.
 

Prep, Post-Procedure & Post-Operative Information

Preparation

Most surgeries and procedures require you to do some preparation to clear the colon.

Post-Operative Instructions

When you are discharged from the hospital you will receive instruction for your at home care. To prepare for your return home please review the appropriate instruction sheet.
