Our Home Care program is designed for patients who are unable to leave their home due to a medical condition. As a patient of Southern New Hampshire Health and Foundation Medical Partners, we are pleased to offer Home Care visitation services to support your care while medically necessary. Our Nurse Practitioner is available to visit you upon discharge from the hospital or on an ongoing basis. They will work collaboratively with your primary care and or specialists as well as your care team to provide care specific to your needs.
Our goal is to allow you to remain in your home but still receive the important health care that you need. Some of the services we provide include:
- Health history and physical examination
- Health assessment
- Co-management of acute and chronic medical conditions
- Preventive and wellness education
- Assessments of activities of daily living
- Psychosocial assessment
A nurse practitioner is different from a “visiting nurse” and your care team may also recommend services from visiting nurses where appropriate. You may also be referred to lab or radiology testing, other homecare services or emergency care should you need it.
To remain enrolled in the Home Care program and maximize optimal coordination of care, you should continue to see your Primary Care Provider (PCP) annually. Your PCP may decide to see you more frequently depending on your clinical needs or may recommend you transition your primary care to a Foundation Community Care Provider.
