- Nashua Rehab Clinic, 17 Prospect St, Nashua – Open Monday – Thursday, 7:00am – 7:00pm and Friday: 7:00am – 4:30pm
- Pediatric Rehab Clinic, 5 George St, Hudson – Open Monday thru Thursday 7:30AM-6:30PM and Friday 7:30AM-4:30PM
- Hudson Rehab Clinic, 300 Derry Rd, Hudson – Open Monday – Thursday, 7:30am – 7:00pm and Friday: 8:00am – 4:30pm
- Milford Rehab Clinic: 10 Jones Rd, Milford – Open Monday and Tuesday 8:00AM-7:00PM, Wednesday and Thursday 7:30AM-7:00PM and Friday 7:0AM-4:00PM
- Merrimack Rehab Clinic: 696 Daniel Webster Highway, Merrimack – Open Monday - Thursday: 7:00am-7:00pm, Friday: 7:00am-5:00pm
